- 92 mutations across 40 files moved to useMutationWithToast
  (success/error toast + invalidateKeys); complex flows left as-is
- 24 boolean modal flags moved to useModal
- Mutations without an original onError intentionally not migrated
  to avoid introducing new error toasts
